Social Worker Full Time - Days United Methodist Communities is currently seeking a Social Worker for our Pitman Community. We welcome you to join us in our small, long term care community! Senior Living in Gloucester County at our UMC senior community in Pitman, New Jersey, is truly integrated into the life of the town. We are part of a beautiful residential neighborhood, near to highways and public transportation with easy access to both Philadelphia as well as the Jersey Shore. The Social Worker overseas the residents, in the skilled nursing setting, by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Requirements for a Social Worker: Bachelor's degree in Social Work (BSW) or Human Service field. Proficiency in a clinical software system. Understanding of the MDS, care plan and admission and discharge processes. One year healthcare/long term care experience. Certified social worker license with the State of New Jersey (CSW). Responsibilities for a Social Worker: Develop, maintain and utilize a listing of current community resources. Advocate for the resident and their family. Understands and meets all requirements for social service documentation, i.e. interdisciplinary care plan; d/c notes: advanced directives; resident rights, etc. Prevents and addresses resident abuse. Educate associates regarding residents' rights including how to recognize and prevent abuse, neglect and mistreatment, resident dignity, independence and quality of life - as outlined in the Resident Rights. Provide the residents, families, significant others and associates with communications regarding care conference such as the date and the time of the meeting, information such as; room/roommate change, physician change, last covered date discharge date.
