Position Details Perform assessments and provide care to patients to develop a plan of care in collaboration with the patient, family and other health care providers. Coordinates services and interventions identified in care plans. Evaluates the patient's response to and progress toward their optimal patient experience and outcome. Committed to patient and family centered care. What You Offer Must be an RN currently licensed to practice in the state of Minnesota. Ability to build and maintain positive working relationships with patients and their families, peers, physicians and other health care professionals. Proficient in the use of electronic medical records systems. Communicate effectively in English, verbally and in writing. Pass a background study through the Minnesota Department of Human Services.
