Chariton Valley Association is seeking a Full-Time Community Registered Nurse (CRN). The Community RN promotes the physical and mental health of individuals by applying the nursing processes as a member of the management team. The position coordinates and monitors medical services, including health and medication services, in accordance with the requirements of regulatory agencies and in response to individual need. The Community RN's will provide at least 1.25 hours of service to each individual supported each month. Additionally, hours of service for Professional Assessment and Monitoring Services (PAMS) may be authorized. The Community RN will become certified to instruct Level 1 Medication Administration and CPR/First Aid training and will deliver training routinely, ensuring all staff have adequate training and recertification as applicable. The Community RN will create a detailed monthly documentation of all services and complete a monthly summary for each individual receiving Community RN or PAMS. Community Registered Nurse (CRN) responsibilities include: Participates in the admission of new individuals to assigned programs. Clarifies initial or hospital discharge medication orders and contacts the physician to review the individuals' status and medication plans. Conducts assessment of individuals within 7 days of discharge from hospitalization or ER visit, including skin assessment. Develops plans to ensure that the medical and health care needs of individuals are met. This responsibility includes: Ensuring appropriate medical follow-up. Providing nursing delegations related to medical diagnosis, allergies, medication, diet and special emergency plans related to ongoing medical conditions. Coordinating and reviewing lab work. Coordinating care with clinics, outside physicians and hospitals. Health and nutrition review and teaching. Medication Administration and other Staff training as assigned. Follows up on health related concerns as identified by the Department of Mental Health RN. Functions as part of the management team attending program staff meetings on a quarterly basis Completes the medical sections of the Individual Support Plans that address health and illness needs and issues. Documents services in accordance with agency policy and requirements associated with Medicaid and Department of Mental Health Certification and Contractual compliance. The Community RN will input information in the state HRST system and update monthly and as-needed. Promotes individual empowerment and self-care activities including self-medication. Ensure all CIMOR and APTS referrals are completed in a timely Supervise Community LPN Effectively communicate in a professional manner, both verbally and written. Participates in on-call rotation with other nursing staff. Skill Requirements: Customer/Client Focus. Ethical Conduct. Personal Effectiveness/Credibility. Thoroughness. Time Management. Leadership Public Speaking/Teaching Written and Verbal Communication Education, Qualifications and Skills: REQUIRED - Minimum associate degree in nursing. High School Diploma or equivalent Current State License as a Registered Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile. Professional CPR Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling and stooping. The employee may need to have the ability to transfer individuals and therefore be able to lift over 20 pounds. Work Environment: This job operates in a professional office environment as well as persons' homes and the community. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. Travel: Daily travel to multiple work locations is required to perform job duties. The amount of travel required will vary day to day. Position Type/Expected Hours of Work: This is a Full-Time position. All Full time positions at CVA offers a full range of benefits including sick, vacation, holiday pay as well as health, dental, vision, life insurance, and supplemental insurance options. Hours of work are typically Monday through Friday 8AM-5PM however, there is an expectation that the Community RN will be flexible with their schedule to meet the needs of the clients supported. This position will also have on-call responsibilities . Preferred Requirements: Bachelor's degree in nursing Certificate to teach Level 1 Medication Aid after meeting state requirements Certificate to teach CPR/First Aid Schedule/Availability: Hourly position with some schedule flexibility. Typical days are Mon-Fri 8am-5pm, with on-call responsibilities and flexibility to teach training courses as needed. Job Type: Full-time Salary: Hourly position. Education and experience are factored in for salary.
