ACT (Assertive Community Treatment) is a highly intensive, outreached based, out of clinic, service provided by a multidisciplinary team of professionals. Population targeted is individuals who have serious and persistent mental illness and most have a co-occurring diagnosis of substance abuse. These are individuals who do not and will not come into the clinic setting for traditional treatment. The ACT Team takes the treatment services to them in their homes, shelters, sidewalks, etc. ACT is available 24 hours/7days a week to individuals through an on-call rotation among ACT team members. Services will be provided afterhours and on weekends. Services provided to Baldwin and Bibb counties. Duties & Responsibilities: Assess client health problems and needs, develop and implement nursing care plans, and maintain medical records Administer nursing care to ill, injured, convalescent, or disabled clients May advise clients on health maintenance and disease prevention or provide case management. Minimum Training & Experience: A current Georgia license to practice as a registered professional nurse. Additional Information: NO differential shift pay is included with this position.
