Title: Intensive Case Manager- Addictions Treatment Description: As a member of our Opioid Treatment Team, the Intensive Case Manager participates in interdisciplinary team meetings, coordinating care with our medical and nursing teams, to help improve the health and wellbeing of our patients. The Intensive Case Manager will assist clients enrolled in the program with accessing community-based services, make appropriate referrals, perform general case management tasks, and complete enrollment, intake, level of care assessments, and new patient orientation under direct supervision. Qualifications: One year as a Drug and Alcohol Case Management Specialist Trainee; OR A Bachelor's Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or a related field; and one-year direct service experience performing counseling or case management work in alcohol or other drug treatment setting; OR A bachelor's degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice or other related social sciences; and one-year direct service experience performing counseling or case management work in alcohol or other drug treatment setting. Or: Any equivalent combination of experience and training. Responsibilities: Conducts intake and orientation for all new patients, including review of the patient handbook, clinical assessments, and level of care assessments. Create initial treatment plans and treatment goals with patient needs identified and incorporated. Based on patient treatment needs and assessment, coordinates and assists with community-based referrals and case management services, including housing and bus pass applications. Attend weekly supervision sessions and weekly treatment team meetings. Performs ongoing data collection for reporting on status of patients, including admissions, discharges, and outreach/ follow-ups. Management of small individual caseload and group facilitation, per direct supervisor approval. Schedule: Full-time, Day shift; 530am-2pm, 6am-230pm, or 630am-3pm. Flexibility with hours due to closing early on Fridays.
