Title: Lead Care Coordinator (Master Level) - Inpatient Description: Responsible for supporting the management team in staffing and workflow of the Care Coordination inpatient social services team under the direction of the Social Services Manager. Assures quality assurance of all assigned team members who are part of the site and/or program. This also includes ongoing training and individual supervision for assigned team members to assure that clinical competency and high-quality client care is being provided. Qualifications: Master's degree in Social Work, Psychology, Marriage and Family Therapy, Counseling or related field License preferred but not required: LPC, LMFT, LSW, LCSW 2 years' experience in social service field with experience in care coordination, discharge planning, or case management Responsibilities: Maintains caseload as needed to support staffing and care coordination needs throughout the department; completes care coordination job duties for assigned cases.
