JOB S U M M A R Y : T he Community Services Case Manager provides direct/indirect client services to eligible individuals/families to empower and educate families/communities to address and overcome the barriers of poverty. M A JOR T A SKS The Case Manager reviews intake, a ss e s s m e n ts, a n d applications of e l i g i b l e i n d i v i d u a l s / f a m ili es to d e t er m i ne th ei r s tre n gt h s/n e e d s and a ss i s t t h em w i th ac c e ss i ng external and internal r e s o u rc es to m e e t t h o s e need s . The Case Manager a ssure a p p l i c a t i o n s f or progra m s are complete and a ss i s t i n d i vi d u a l s/ f a m ili es w i th d o c u m e n ta ti on needed to c o m p l ete the application to r e m o v e b a rr i ers a n d a cc e s s progra m s for training, education, or emergency services . E DU C A T I O N A ND E XP E R I E N C E R E Q UI R E M E N T S : B achelor's degree in healthcare, business, nonprofit, or related fields. 2 years of experience will be considered in place of education. E x p e ri e n c e i n r e l a t ed f i e l d s u c h as w or k i ng w i th e l d er l y , d i s a b l e d , a n d /or l o w - i n c o m e i n d i v i d u a l s pre f er r e d . Ca s e m a n a g e m e n t e x p e ri e n c e p r e f er r e d . Must attain Family Development Credential (FDC) and CMS Navigation Certification within first year of employment.
