Trinity is currently hiring a full-time Family Support Specialist, within our Community Network Services Division. Responsible for ongoing management of an average caseload of 15 children and families. This includes goal planning, development and keeping case records, planning and advocating of services to assist families, administering the Ages and Stages Questionnaire Screenings as indicated and other responsibilities as designated. What will you be doing as a Family Support Specialist? Responsible for a caseload of an average of fifteen (15) children and their families, to plan long and short-term goals for the child and family. Write up weekly goals and objectives and select appropriate methods and materials for implementation. To develop and keep a weekly case record on the interactions between worker, parent and child and how the objectives are being met. Responsible for keeping a record of all client contacts. Participate in on-going case management, planning and advocacy services for families. Participate in client case conferences, to serve as ad advocate for client in obtaining and/or maintaining supportive services. Function as a mandated Child Abuse and Neglect Reporter Prepare all released and consent forms. Responsible for administering timely developmental screenings for program participants. Responsible for submitting all necessary statistical data and forms required by funding source. Responsible for administering participant satisfaction surveys. Keep staff informed of family program status. Write brief weekly reports and quarterly reports for Program Director Attend on-going, job related trainings. Assume other job related duties as assigned by the Program Director an/or agency CEO What are your skills and experience? BS in Education and/or Human Services preferred or an AA degree with related experience working with young children and their families. Knowledge and experience working in the field of Developmental Disabilities preferred. Considerable work experience and professional training may be substituted for educational requirement.
