Case Manager, Full Time (40 hours) St. John's Hospice, Philadelphia, PA Summary: St. John's Hospice, a premier men's shelter, has a full time opening for an energetic and compassionate Case Manager. Our staff is passionate about working to provide the best quality services, with dignity and respect, to the homeless and underprivileged in Philadelphia. The right person will join a small and dynamic team and be comfortable working directly to provide services and guidance to the individuals in residence. Best candidates will have a minimum of a BA degree and two years of experience working with victims of trauma, mental health, substance abuse or homeless services. This is an opportunity to make a difference. Principle Responsibilities: Develop a comprehensive service plan, based on identified need that includes goals, action steps and appropriate timeframe for completion. Monitor and aid individuals as needed. Small group and supportive counseling skills a must. Data entry and computer literacy required. Network with outside agencies Other duties as assigned KNOWLEDGE, SKILLS AND ABILITIES: Computer, and typing skills. Knowledge of homelessness, substance abuse, and mental illness preferred Good written and verbal communication skills EDUCATION: BSW or comparable Bachelor's degree EXPERIENCE: Two years social work experience PHYSICAL REQUIREMENTS: Ability to stand, sit, walk, climb stairs, operate controls, reach out with hands, occasionally lift objects up to 40 lbs.
