Registered Nurse- Care Manager - Bilingual Bengali/Urdu VillageCare is looking for a self-motivated and passionate RN for our Full-Time Care Manager opening. This is an exciting and dynamic position, as the Care Manager, you will be responsible for ongoing assessment of health, environmental, and psychosocial status of members assigned to you, and ongoing care management of members through telephonic contact and occasional field visits for chronic cases. Some of your daily activities will include all forms of care management, such as- care planning, coordination, and assessment of health. You will monitor services for quality, effectiveness, and progress toward goals. Coordination of transitions for patients including discharge planning from the hospital or nursing home. Identifies health, environmental, and psychosocial risks for assigned members and recommends appropriate interventions to the Interdisciplinary Care team. We would like to speak to those who have a valid NYS RN License along with 3+ years job related experience in Managed Care, Home Care or Community setting. Seeking those who bring an Associate's degree or higher. Passionate attitude, communication skills and problem solving will be vital to this position. Bilingual - Bengali/Urdu required.
