Be a part of a great team that helps people facilitate their mental health and/or substance use recovery and achieve their highest level of stability and independence through the Comprehensive Community Services (CCS) Program. CLA has a strong and highly supportive Community Wellness Solutions Team that works together to meet the needs of our participants, their families, and our staff. CCS is a Medicaid funded program that provides mental health and substance use services to people of all ages. (CLA works with 18+) The intent of CCS services is to assist people in identifying their personal goals and in working towards those goals at their own pace. Once participants have developed a Recovery Plan in collaboration with their Recovery Team, their Service Facilitator will assist them with identifying the service providers they would like to enlist to provide the services they need. In many instances they will have the ability to choose from several available service providers. NO WEEKENDS OR HOLIDAYS Part-Time (less than 30 hours/week) Full-Time (30-37.5 hours/week)Benefit Eligible Starting Wage: $21.00/hour, b ased on individual's education and experience This position requires work to be completed in client homes and in an office setting (Dane County, WI area), with the possibility of completing some work remotely once job-competency has been demonstrated. This is a clinical mental health position that offers the opportunity to complete training hours and includes access to regular, clinical supervision. Primary Duties: Complete comprehensive assessments with CCS participants in collaboration with relevant supports to gather history and identify strengths, needs, and barriers. Use a strengths-based and person-centered approach to support consumers in identifying goals and creating their recovery/treatment plan. Maintain regular contact with consumers to monitor progress toward goals and assess for additional/changing needs. Provide ongoing support and case management to a caseload of 15-20 consumers. Manage deadlines, including annual and bi-annual assessment and recovery plan updates. Maintain documentation of consumer interactions. Review team member (providers') documentation to monitor services and ensure alignment with recovery goals. Applicants must have experience or be comfortable working with diverse populations including those from low socioeconomic status and those experiencing different levels of mental health challenges. Minimum Education and Experience: Bachelor's Degree in Mental Health, Human Services, or related field, required; Master's Degree strongly desired Comprehensive Community Services (CCS) credential in Dane County WI, strongly desired 2 years' experience in Case Management and working with individuals with mental health conditions, intellectual disabilities, physical disabilities, chronic health conditions, or seniors, strongly desired Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.
