Visiting Nurse Association is looking for a full-time or part-time nurse practitioner with palliative care, hospice care or geriatric care experience to make visits to patients in their homes. This is an excellent opportunity to provide quality one on one care for patients who are seriously and chronically ill.Our team works together to support our patients to provide the best experience possible. In-home palliative care works in collaboration with the patients primary care physician and helps to reduce hospital readmissions and emergency room admissions.Qualifications: 2 year of palliative care, hospice care or geriatric care experience. Candidates with similar backgrounds will be considered. Current NP license to practice in the state of Missouri required. Excellent communication, writing and computer charting skills.
