If you want job flexibility and to make a difference in the community, join Acenda's Integrated Health Team as a Peer Support/System Navigator for our Early Intervention Support ServiceYou will be joining a team focused on healing our communitiesAcenda is an integrated health care agency dedicated to industry-leading prevention, treatment and wellness services compassionately delivered to ensure that every individual, family and community achieves their greatest potential.What you'll do- Work with the Psychiatric Emergency Screening Team to ensure individuals needing assistance achieve mental wellness through a Wellness Recovery c Action Plan (WRAP)- Operate as a strong community liaison, and complete face to face follow up for the individuals' exit from inpatient care, to ensure that individuals can navigate and understand all services which are available in the community-Complete crisis planning as needed to decrease future hospitalizations and increase quality of daily life- Collaborate with Acenda and Community Providers to link individuals into services needed.What we provide- Flexibility with your work setting and hours- Career advancement opportunities including participation in an Acenda leadership development program- Year-end performance bonuses- CEU's and free training programs- An innovative culture which allows you to grow and learn with the agencyWhat you'll bring to the team- Minimum of a High School Diploma-Preferred Certified Peer Recovery Specialist (CPRS)- Lived and personal experience with mental health and substance use to provide support to the persons we are engaging.- Excellent personal communication skills in order to motivate and engage individuals, as well as being effective with linking to necessary treatment services- Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record- Professionalism and a commitment to excellence in careTogether we'll be:- A passionate, innovative, care-focused team, working collaboratively and creatively to ensure the health and wellness of individuals and families in our communitiesEEO Job Posted by ApplicantPro
