BEING A REGISTERED NURSE (RN) - CASE MANAGER: YOUR IMPACT As a Registered Nurse (RN) - Case Manager, you go above and beyond to make sure our patients are well taken care of! Every day brings something different as you skillfully design and manage individualized treatment plans. Depending on each patient's needs, you create custom treatment plans and set medical appointments accordingly. If a patient misses an appointment, you reach out to them to find out why and to reschedule a future one. You meticulously maintain daily office schedules and admission lists, making special note of any out-of-area patients. When a new patient joins our clinic, you ensure they complete new member orientation. You administer other tests as needed, such as Mini-Mental exams, psychosocial assessments, and activities of daily living (ADLs) screenings. Working in conjunction with hospital and nursing home teams, you help coordinate services for admitted patients as well as notify facility staff of any possible admissions or ER diversions. You carefully review inpatient lists for the appropriate facilities and diligently maintain all patient documentation, making note of status changes and discharges. As you track patients, you identify any that need a disease management program and follow up with them according to protocol. When appropriate, you help review pre-op clearance as well as review inpatient and outpatient procedures. You enjoy performing this invaluable administrative job and take great pride in connecting the people in our community with great healthcare! WHAT WE'RE LOOKING FOR IN A REGISTERED NURSE (RN) - CASE MANAGER Current RN certification Accredited Case Management (ACM) OR Certified Case Management (CCM) licensure OR ability to earn a case management certification CPR OR BLS certification Geriatrics OR critical and intensive care medical specialties Primary care case management experience would be preferred. Experience with Medicare Advantage would be a bonus! Are you highly organized and detail-oriented? Can you effectively manage multiple tasks without letting one fall behind?
