Job Description: As a Home Infusion Nurse Supervisor(RN) you will practice your leadership and clinical skills in a growing specialized area of the nursing field providing a broad range of infusion therapies in our patient homes and infusion suite. Ensure the nurses successfully communicate all clinical issues to the appropriate healthcare team members and monitor compliance of nursing-related documentation and plan of treatment orders. Implement new policies and programs and collaborate with the pharmacy team to develop, monitor and evaluate goals of patient therapy. Support the sales team as needed with new clinical programs and/or marketing activities to physicians, nursing agencies, and referral sources. Provide safe, direct patient care utilizing the nursing process in the ambulatory setting. Provide clinical mentor-ship to staff. Provide full range of patient education to patients and family members. Ensure adherence to departmental policies and procedures. Patient care assignment may include pediatric, adolescent, adult and geriatric age groups. Accountability: PATIENT CARE:· Assess patient's condition and nursing needs; set goals and utilize appropriate nursing actions to achieve goals.· Provide and document nursing care.· Ensure that physician's orders are carried out.· Assist team members in patient care; coordinate patient care in conjunction with Pharmacy staff and Providers.SUPPORT:· Ensure that equipment is maintained; monitor supply and medicine requisitions; maintain supply stock levels.· Ensure patient medical records contain necessary information; document medical history or chief complaint.· Obtain and document lab results; ensure follow-up on abnormal results.· Record and report pertinent observations and reactions regarding patients to following Providers.PATIENT EDUCATION:· Provide education for patients and/or families about infusion services, preventive care, medical issues and the use of medical supplies and equipment.PATIENT SAFETY:· Follow patient safety-related policies, procedures and protocols· Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes· Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk· Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner· Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right" Certification Requirements: · Complete and maintain required certifications and competencies as listed in the department expectations/and or the unit/clinic education matrix Education Requirements: · Nursing program (nationally accredited) graduate Licensure/Certification Requirements: · Licensed Registered Nurse (RN) in State of New Mexico· CPR Certification for Healthcare Providers or for Professional Rescuers· Minimum two years nursing experience with a minimum of one year infusion/IV experience.· Must possess a valid and current driver's license and auto insurance. Experience Requirements · 1 years directly related experience to IV infusion. Experience preferred but not required: - Pediatric Care- CRNI Certification Physical Demands Requirements: · Heavy Work: Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects or people. Physical Demand requirements are in excess of those for Medium Work.
