The Position: We are currently in search of a Coastal@Home Care Navigator. The Navigator's main responsibility includes supporting the Coastal@Home team in caring for patients enrolling in and referred to the program. This is an hourly, full-time position with a schedule of 40 weekly hours, Monday through Friday. This position is located at our Corporate Office in Providence, RI. Employee parking is provided. At times, this position may require travel to and entering patient's homes with a clinician. Essential Functions: Works closely with the Nurse Care Manager through telephonic outreach to patients based on risk stratification for routine follow up and emergency room Transitions of Care outreach Performs telephonic patient engagement, triage and related tasks including basic patient histories, medication reconciliation and coordination with care team members Assesses patients for medical and social needs and intervenes when appropriate within the scope of practice Facilitates and reinforces patient self-management and encourages adherence to PCP appointments, lab work, diagnostic testing, specialist visits and care plan goals under the direction of the clinical team Collaborates with the clinical team to help patients reach their goals, maintain their health and improve their quality of life Requirements: High School Diploma or equivalent Medical Assistant Certification or equivalent clinical training At least 1 year of experience related to care coordination or patient engagement including previous CNA or home care experience Previous experience with an EMR Why You Should Apply: We operate in a collaborative environment and strongly encourage and promote employee growth and development.
