A DAY IN THE LIFE OF THE STEP CASE MANAGER As the STEP Case Manager, you will have a rewarding job doing the following: Assessing the needs of up to 10 justice involved youth who are moving onward to or returning to stable, supportive community or family placements and providing case management check ins, crisis interventions, family mediation, service coordination, educational support and life skills development as needed/determined. Working in partnership with justice involved youth and their teams to create treatment plans and complete comprehensive assessments to help them reach their goals while in program and ongoing when they depart Advocating for and obtaining services for youth and families through various community, county, state, and federal programs, including educating youth and their families in self-advocacy, eligibility, and other relevant information Maintaining ongoing communication with a network of community partners and youth treatment team partners to support each youth's progress and tracking of needs Maintain and update all necessary databases for tracking of youth progress, program objective and outcomes, and managing the needs of each youth Making a difference in the lives of young adults gives you an incredible sense of purpose and that is why you love your job! QUALIFICATIONS Bachelor's degree social work, psychology, sociology, or a relevant field with 1 year of experience, or a Master's degree in one of the above fields. Must have demonstrated knowledge and understanding of systems of care for youth and families including child welfare, juvenile justice, mental health, and substance abuse and treatment.
