Assist family worker with the development of treatment plans that take into account the child's medical needs. Provide medical information and training for foster caregivers, biological parents and staff. Attend at least one medical staffing per year on each child served under the Medically Complex Foster Care program and all other DCFS contractual obligations. Monitor the medical care of the child and document it in the client's file. Conduct an in person contact with the child, foster caregiver and family worker at least once per quarter or as needed. Actively participate in child and cagily team meetings. Conduct in person contacts every 30 days with children experiencing the following conditions: Requires a G-Tube, failure to thrive; ventilator dependent, or any other unstable medical condition; or children recently discharged for the hospital until stabilized per doctor's orders. Recently released children who are stabilized per doctor's orders will require an in person contact every 60 days. Provide medical consultation and education for foster caregivers, biological parents and staff for non-medically complex children as needed. Assist with scheduling of psychiatric consultation with contracted psychiatrist. Qualifications: A Bachelor's degree, and LNP or RN license (Required) Experience in nursing or foster care related position (Preferred) Completion of DCFS criminal background check (Required) Must be at least 21 years of age, obtain an insured vehicle, clean driving record, and three years driving record with a valid driver's license For information on The Center for Youth and Family Solutions, including more information on employee benefits and our company culture, visit our website at https://www.cyfsolutions.org/ or check out our video campaign here .
