Job Summary The Clinical Liaison Lead (Lead - CL) assumes the role of content expert for the Clinical Liaison Team providing support for facility and hospital liaisons, information to the Intake Team, and IDT. The Clinical Liaison Lead represents the agency in the hospital or facilities (as needed) while demonstrating sensitivity and compassion for patients and families facing life limiting illness, death, and bereavement. The (Lead-CL) supports hospice functioning within the hospital setting including consultation, consents, coordination and communication with patients, families, case managers and physicians. The (Lead-CL) will follow HSCC policies and procedures related to the GIP level of care and maintain systems to ensure hospice is compliant with all regulations. Essential Responsibilities Facilitates admissions by consulting with patients and families to initiate the process in the hospital prior to transfer home or to a facility. Obtains and reviews medical records and other supporting documentation to determine hospice eligibility. Meets with patients and families to explain hospice services and payment. Offers Information and Registration visits for hospice, palliative care or Transitions as needed. Facilitates transition to home in conjunction with hospital discharge planners as indicated. Documents all info, consents, and admissions in the hospice clinical record. Facilitates, coordinates, and manages admissions to the general inpatient (GIP) level of care for hospice patients. Provides GIP admission, oversight, and visits to hospitalized patients receiving GIP level of care. Documents and communicates care provided in an accurate, complete, and timely manner, reflecting the nurse's role as case manager. Meets with patients and families to explain hospice services, GIP level of care, and payment. Completes paperwork associated with direct admissions from acute care to the hospice general inpatient level of care per hospice and hospital protocol. Provides education for hospital professionals about hospice eligibility and HSCC services as well as regulations and symptom management. Collaborates and prepares current presentations as needed to hospital staff and physicians. Provides individual consultation regarding palliative/hospice care as needed. Communicates with case managers regularly regarding hospice consultations. Coordinates payroll, scheduling workflows for the Clinical Liaison Team Participates in QAPI program. Acts as agency resource for expertise in GIP level of care. Maintains proficiency with clinical documentation system and QAPI reporting. Coordinates annual hospital contract evaluation meeting and follow up. Maintains personal professional growth, focusing on communication skills, hospice clinical knowledge, and hospice eligibility criteria. Maintain membership in at least one nursing professional organization. Attend HSCC internal in-services as directed. Maintain current knowledge of the Medicare Hospice Benefit, billing issues, regulations and compliance, palliative care, and end of life care. The nurse participates in other agency activities as assigned or as indicated. Attend monthly team meetings, general staff meetings, and committee meetings, and IDT as needed. Participate in agency sponsored events or special projects. Admit patients to hospice service when needed. Compliance Ensures compliance with all job-related State/Federal Laws; ensures all HIPAA Privacy and Security Regulations, Medicare Conditions of Participation, and all other regulatory/compliance requirements are understood and followed. Requirements Education/Experience Graduate of an accredited school of nursing, BSN preferred. 3-5 years of hospice RN Liaison work, required. 1-2 years of hospice or home health nursing experience preferred. Experience in customer service preferred. Bilingual strongly preferred. Skills Required Solid understanding of all regulatory and legal requirements. Excellent verbal and written communication skills. Excellent presentation skills. Ability to develop and sustain effective working relationships. Strong computer skills in Microsoft Office and competence using electronic medical records systems. Critical thinking and ability to function both independently and as a member of the Admissions team. Certifications, Licenses, Registrations Active California RN License. Current California Driver's License. Certification as Hospice and Palliative Nurse (CHPN) preferred. ELNEC training preferred. Pass OIG/GSA review and all required background checks. Physical Demands Strength to lift and carry materials weighing up to 10 pounds. Vision to read printed materials and a computer screen. Hearing and speech to communicate in person and over the telephone. Work Environment/Environmental Condition Must possess mobility to work in a standard office setting and to use standard office equipment, including a computer.
