Job Title Program Facilitator - Early Memory Loss Program Department Adult Day Care Supervisor Job Objective Conduct daily programming and provide therapeutic recreation assistance to EML clients. Minimum Qualifications Education- Associate Degree in Therapeutic Recreation, Human Services, or related Therapies Experience- 2 or more years of therapeutic recreation experience or experience working with individuals with Dementia diagnosis Licensure- None Part-time: M, W, & F Flex 15-20 hrs. per week (Days) Job Standards Interact in a professional manner at all times, exhibiting friendly behavior when working with clients, family members, collaborators, the general public, and co-workers Report to work each day at scheduled time and adhere to lunch and/or break schedule Complete all mandatory trainings each year Self-motivated and well organized Strong and confident communicator Perform other duties as assigned Patience and understanding of behaviors exhibited by this population Ability to maintain calm in the event of emergencies Ability to work independently and as a team player Ability to handle multiple demands and tasks Essential Job Functions: 1. Assess potential clients for the appropriateness of the program Create and or implement daily agenda, research, plan and create appropriate therapeutic activities and outings in coordination with the EML Program Coordinator 3. Encourage participation of all clients through motivational approaches. 4. Assist in developing, implementing and leading Recreation Programs in collaboration with the EML Program Coordinator. Participate in client recreation assessments, care planning and routine charting as needed. Report observable changes in client behavior or physical status to EML Program Coordinator. Prepares in advance for planned activities which includes, but is not limited to, acquiring necessary materials and setting up the room. Assist other staff as needed to set up room. Clean up activities and return to storage areas and maintain activity areas in a neat and clean manner. Oversee the daily operations of the Early Memory Loss Program when the Coordinator is unavailable to do so. This includes but not limited to facilitating in-person programming and or virtual sessions. Assist with activities of daily living under direction of EML Program Coordinator, which may include serving morning coffee, serving lunch, ambulation assistance, and transporting client to therapy as needed. Assist clients in and out of the building at pick-up and drop-off times. Maintain regular attendance and perform other related duties as assigned. Non-Essential Functions Conduct program tours to visitors and assist in referral intake as needed.
