Evaluate job readiness and/or education readiness/level. Provide employment readiness services & assist with job and career advancement education and skills. Coordinate services holistically with Individual Coach. Maintain effective communication with participants to ensure employment and economic goals of their Individual Development Plan are achieved. Flexibility required for potential nights & weekends to accommodate outreach/workshops/intakes. DUTIES AND RESPONSIBILITIES: Conduct initial/ongoing assessments of participants to determine strengths, needs, interests, skills, goals and resources to determine job/education readiness and economic stability needs. Create/monitor each participant's individualized employment plan, in conjunction with his/her Individual Development Plan to ensure all goals are achieved; document progress as needed in nFORM. Deliver mentoring, coaching & providing education to build awareness of and access to job search tools/forums, resume building, cover letter writing, interview skills, employment skills, money management skills, provide as needed. Conduct employment workshops for clients/participants as assigned. Conduct outreach to recruit/engage potential clients & community organizations serving the target population, presentations to community providers and government entities. Support Data Manager with CQI tasks related to the program, including: participant data progress tracking, data sharing with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress, work with the entire CN Guidance team to apply the Plan-Do-Study-ACT continuous quality improvement approach to address systemic, organizational challenges, complete QI Training during onboarding and participate in QI refresher training as needed. Assist Evaluation Team with research elements of the program and data collection in relation to the local and federal evaluations, including nFORM surveys. Conduct relationship workshops using evidence-based curricula. Provide support to participants around workshop content as needed both in the office and in the community. Participate in team meetings as scheduled. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress Complete QI Training during onboarding and participate in QI refresher training as needed. Additional duties as assigned. EDUCATION AND EXPERIENCE REQUIREMENT: Bachelor's degree in human services required; Master's degree in human services preferred. Experience in a mental health related field, coordination of services, employment/education services, collateral contacts; Bilingual in English and Spanish preferred.
