The First Five program will provide trauma-informed care to support children and families by strengthening and enhancing services in early learning centers. First Five has an overarching goal of helping early learning center teachers create a trauma-informed culture in their classrooms. Through classroom observation, First Five identifies children needing behavioral support. There are two pathways of service depending on the level of complexity and risk of expulsion that a child is experiencing. Position Summary: This is a professional position that will provide Integrated Decision Team (IDT) Facilitation to families whom are receiving services through the First Five program. Upon the family opening services with First Five and assignment to the ICM, the IDT Facilitator will schedule, coordinate and facilitate all necessary staffing's at a minimum of every 15-days, and maintain staffing schedule throughout service provision. The IDT Facilitator will lead the discussion that will identify the needs and resources for the family and will drive the service plan established to ensure the ICM is facilitating a service plan to improve the child's behavior and increase the child's capacity to learn. The Integrated Decision Team Facilitator coordinates and facilitates the discussion around the child's development, learning, educational, and behavioral health needs and identifies options, objectives and goals and facilitates the team to arrive at a shared decision on the best approach with the family to ensure needs are being met and maintained. The IDT facilitator will ensure that the family is in attendance of every staffing. This position is required to complete entries into the agency electronic health record. DFL prides itself on the fact that it is a trauma informed care services provider. This position requires that trauma informed practices are embedded into all services provided. This position is field based, IDT Staffings are to occur from the classroom, in families' homes when possible, and at minimum at a mutually identified place, where the family feels most comfortable. Hours are flexible, evening and weekend work is required. This position will work closely with members of the First Five team such as: Counselor, Intensive Case Manager, Clinical Consultant, SME, and Supervisor and in collaboration with other community service providers externally to provide complete comprehensive services. Responsible to meet productivity requirements of 112 billable hours monthly. This is a field based position. Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. Essential employees will be required to work in the field, and or be "on call" depending on the needs of the community during the time of an emergency. EDUCATION EXPERIENCE/REQUIREMENTS: Bachelor's Degree in psychology, social work, criminal justice or related area of study from an accredited university. Master's degree in the above identified fields preferred. Two (2) plus years of experience working with vulnerable populations to include those within the early education or child welfare system. Must acquire knowledge of community resources. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly. Must have the ability to learn Avatar within the first 30 days in the position. This is a condition of employment. Must be able to use Telehealth platform for service delivery. Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage and pass the annual vehicle inspection. Must be vaccinated against COVID-19 or be willing to be vaccinated against COVID-19 immediately upon hire. This is a condition of employment.
