Community Housing Specialist General Description: The Community Housing Specialist is responsible for providing rehabilitation services to individuals recovering from mental illness and substance use disorders. The Community Housing Specialist works with individuals in their supportive housing or Continuum of Care residences and in the community. This position works alongside outside service providers to ensure that services are in place for individuals to develop the skills necessary for independence and success. Schedule: Full time. This person reports to the Psychiatric Rehabilitation Program Director. Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions: A Bachelor's Degree plus two (2) years experience in mental health/psychiatric rehabilitation is required or an AA degree with at least three (3) years experience in mental health/psychiatric rehabilitation may be considered. A background in housing services is preferred. A good driving record is essential, with no more than two points. Leadership qualities and the ability to work as a part of a team are necessary. Ability to respect individual choice in the recovery process and a commitment to psychiatric recovery is required. This position requires excellent verbal and written communication skills and the understanding to respect the abilities and limitations of consumers. Microsoft Office Suite and other computer application for daily business operations and documentation of provided services required. Summary of Essential Functions, Responsibilities and Duties Assist individuals, at least weekly, in developing and carrying out goals identified in the Individual Service Plan. Monitor and document progress in the electronic health record (EHR). Coordinate services for individuals served including Psychiatric Rehabilitation Program (PRP), mental health services, somatic and housing needs. Responsible for PRP paperwork to include annual chart documents, ISPs, Monthly Progress Notes, daily contact notes and Continuum of Care Housing annual renewal packets. Communicate effectively with internal and external providers to ensure linkage of resources and services. Ensure income and benefits eligibility for entitlements on a regular basis. Provide off site services in residents' homes and other community locations as agreed upon with individual served. Responsibility for specialized services and needs such as MTA and ID cards. Engage in crisis management. Act as point of contact for client during hospitalization, rehabilitation, etc. if needed and support any follow-up required. Utilize electronic systems including EHR and payroll systems to complete and verify services, Time and Attendance, etc. Perform other duties as requested by supervisor Other physical and environmental requirements: The Continuum of Care Housing Specialist is required to operate a motor vehicle most days to provide services to individuals living in supportive housing and/or Continuum of Care (COC) housing. Driving will include areas in and around Baltimore County, Baltimore City and Carroll County. While in individuals' homes the Specialist may be exposed to pet dander and other environmental pollutants common in a home setting. Position requires frequent walking, sitting in a car for long periods and walking up and down stairs. There is minimal lifting, nothing more than 10 pounds. FLSA: Non-Exempt
