Join C ommunity Services Group (CSG) , a Certified B Corporation, as a Licensed Practical Nurse (LPN) for our Adult Intellectual & Developmental Disabilities (IDD) Intermediate Care Facility (ICF) in Willow Street, PA and find meaning in your work every day. Hiring immediately for this position. CSG is offering a $1,500.00 Sign-On-Bonus for Nurse positions at our Willow Street location that will be paid after successful completion of the initial evaluation period. This position offers: Starting at $29.00 per hour based on experience. A great opportunity to work outside of a traditional medical office, hospital, or nursing home setting. A small setting where you are able to build relationships with the people you support. The ability to work 1 on 1 with individuals. The opportunity to attend community outings with individuals and teammates. Current Schedules: Full-Time and Part-Time, first and second shift positions available Thursday - Sunday. As a Licensed Practical Nurse (LPN) , you perform nursing work in the care and treatment of adults with IDD and co-existing medical conditions at the ICF, which may include physical health issues/injuries and difficulty meeting their daily living activities. You will also: Evaluate, observe, and assess individuals' physical health conditions. Work with and coordinate care with pharmacy, physicians, and other health care providers. Assist with any treatments prescribed by physician. Ensure people receive error free medication at prescribed times. Provide education on medication, physical health, psychiatric health, nutrition, and selfcare. Educate coworker's on any needed physical health matter for consistency in care. Train staff to correctly carryout physician's orders. Provide medication/health teaching as needed. Provide First/Aid/Acute care as needed. Schedule and attend medical appointments. Provide direct care as needed. Qualifications: A valid Licensed Practical Nurse (LPN) license in the state of Pennsylvania. 1 year of nursing experience preferred. Experience working with people with IDD preferred. Willingness to accompany people into the community and provide support and assistance in interactions while maintaining sensitivity to the rights and dignity of the individual. Ability to work flexible hours. Valid driver's license and the willingness to transport people within the community in personal or CSG vehicles, including passenger vans.
