Care IV Home Health is expanding our Remote Patient Monitoring Program (aka Telehealth nursing) for our home health patient population. To support and grow this key initiative, we are seeking a Registered Nurse to join our us remotely. Great opportunity to work from home, but still utilize your skills! Work schedule is Thursday through Sunday. We would ideally like you to work 32 hours per week. May consider 24hrs if so desired (can shorten weekend schedule). Duties & Responsibilities for Telehealth: Utilize technology (phone, video, etc) and telemedicine to conduct nursing and deliver quality care to our patients in the comfort of their homes Remotely monitor patient conditions and stats such as heart rate, blood glucose levels, oxygen levels, etc. and provide follow up to address, report, and resolve any related issues Coordinate reporting of data and patient status with Directors, Case Managers, Physicians and other relevant medical team members Work closely with patient care teams to provide timely intervention and decrease otherwise avoidable hospitalizations In non-emergency situations, instruct and educate patients on how to address their care needs (proper medication administration, wound care, diabetes management, and other non-emergency medical help) Document all calls & communications, providing data / metrics for company quality initiatives Maintain patient confidentiality while following HIPAA guidelines at all times Additional duties as assigned Required experience for Telehealth: We are seeking Registered Nurses (RN) whose license is active and in good standing with the their State Board of Nursing (need to have a compact license if you are not licensed in Arkansas) Minimum 2 years of nursing experience Preference given to RNs with experience in telephonic nursing & case management Background in cardiac nursing, ICU, or other critical care setting strongly preferred Home Health experience preferred Registered Nurses - Apply online today!
