Hours: Able to work evenings and weekends to facilitate activities. Must be able to work two weekend shifts in a month. Responsibilities include: Development and facilitation of both individual and group activities and events. Networks with Individuals, Program Managers, and Staff to identify great opportunities both in house and in the community. Responsible not only for singular events, but also in helping establish monthly and routine events. Develops partnerships with community entities and connects them with individuals who are interested in their services. Work is primarily in the houses, some work from home or work from office work upon approval.POSITION PURPOSE:The Recreation Activities Coordinator is responsible to assist the Program Manager with the facilitation of community integration and person centered activities for the site. The Activities Coordinator is responsible for the development and facilitation of these activities according to the needs of the individuals at the site. The Recreation Activities Coordinator with work the Direct Support Professional team by providing communication, directions and other instruction needed to facilitate outings which transpire when the Activities Coordinator is not present.NATURE AND SCOPE:The Recreation Activities Coordinator reports to the Program Manager or Program Coordinator, who reports to the Assistant Program Director, who reports to the Program Director who reports to the Vice President of Community Services, who reports to the President. This position requires working every other weekend and holidays, as scheduled. This is a non-exempt position.Essential Functions of the Job- Recreation Activities CoordinatorConnecting individuals we serve and PRI with our community through volunteerism, integration, events, partnerships, memberships, etc.Provide opportunities for new experiences in the community or home.Transport individuals (adhering to all safety policies and laws) as needed to meet program objectives and client or family needs.Create memorable moments that create a positive impact for our community.Effectively coordinates with team members to facilitate community integration and activities through written and verbal communication.Follow, implement, provide, inputs feedback with person-centered and behavioral outcomes.Plan, perform, or assist with cares or activities with individuals according to their preference or needs, to include, but not limited to: household chores (cooking, cleaning, personal hygiene, laundry, dishes, vacuuming, fixing bed, household maintenance, etc.), community involvement, social, recreation and leisure skills, communication skills, personal safety skills, appropriate behavior.Focus on the individual rather than the disability.Maintain professional boundaries between, team members families and individuals served.Displays leadership, serve as a mentor and coach to individuals. Provide guidance and direction while being a positive role model to the individual.Problem-solve issues regarding client situations, with or without assistance.Be creative by trying new ideas that create a more positive impact for the individuals we serve, team-members, PRI, and our community.Maintain confidentiality in all aspects of the job.Ensure the safety and well-being of the individual(s) served at all times.Assist individuals with self-advocacy skills and promote independence.Support new team members by providing on-the-job training.Exhibit flexibility to changing situations and be able to adapt as necessary.Participate in required meetings.Take direction and initiate actions that will allow for cross-training at multiple sites if required.Build relationships by facilitating friendships, partnerships, and personal and professional relationships.If applicable to the person, assist or perform personal cares, bathing, bowel and bladder cares, showering, exercising, range of motion, feeding (oral or feeding tube), hair care (brushing, washing, styling, shaving) dental care (brushing, flossing, dentures) or nail clipping, etc.Medication management, if applicable to the individual served. Follow the six rights to ensure proper medication administration. Ensures medical appointments are attended, physician orders and medical protocols are followed.Display patience when working with those we serve, guardians, and co-workers.Assist with teaching, preparing healthy meal plans to encourage optimal health.Accurately complete, maintain, and submit all documentation and written records as required by the department and/or Rule 10, Positive Behavior Support Plan, Rule 245D or other rules, regulations, and policies or procedures of the agency.Attend workshops and complete ongoing training, as required.Prioritize multiple tasks according to need and the ability to multitask under pressure.Look for ways to promote and improve the quality of PRI services.Complete progress notes/flow charts, task analysis/active treatment, according to outcomes and supports of individuals CSSP.Maintain accurate financial management and documentation of individual funds.Ensure routine and emergency medical services are provided.Assist individuals in the development and implementation of Person Centered Training Packets.Maintain accurate financial management and documentation of individual funds.Ensure routine and emergency medical services are provided.Assist individuals in the development and implementation of Person Centered Training Packets.Assist with ensuring van maintenance is completed.Positively support and promote PRI Wellness in Action program to individuals served.Adhere to PRI Guiding Principles of Person Centeredness, Teamwork and Professionalism by maintaining confidentiality, having a positive attitude, treating others with respect, being flexible, accepting responsibility and utilizing open and effective communication.Be a positive mentor and (through training) coach for new team members to ensure their job success and future.Other duties as assigned.QUALIFICATIONS:High School diploma or equivalency preferred.Must be 18 or olderOne year of experience working with individuals with physical and/or developmental disabilities or human services related field, preferred.Excellent communication skills, verbal, and written.Ability to coordinate activities and services.Ability to read, write and speak the English Language in order to adequately interact and respond to the individuals and meet their service needs.If driving, able to pass the van driving class within first sixty (60) days of employment.If driving, holds valid driver's license and maintains a safe driving record.Ability to be a Trained Medication Passer, within first sixty (60) days of employment.Ability to be trained in CPR and First Aid within first sixty (60) days of employment.Ability to see and hear adequately in order to make assessments and respond to individuals' needs.Ability to respond to emergencies according to established The Phoenix Residence policies and procedures.Ability to use a computer for required documentation, communication and time log purposes.
