Youth Worker : Long Term Health Conditions Job Type: Fixed Term Contract for 12 months Location: Rotherham Hours: 37 hours a week. Some evening and weekend work may be necessary to carry out the duties of this post. Salary: A GBP 26,000 per annum Youth Work helps change lives. Our client is a consortium of 28 VCS organisations established in 2003, recognising they are stronger together in changing lives. The organisation is delivering an exciting new pilot and want exceptional Youth Workers to help them demonstrate what value Youth Work can bring to the topic of Health. Working with Children and young People with long term health conditions (Epilepsy, Asthma, Diabetes) in both the Hospital and Community settings you will be delivering a high: quality youth work program to improve resilience, transitions and change the lives of young people. They have the equivalent of five positions available : one Youth Work Manager (A GBP 32,000 PA) and four Youth Workers (A GBP 26,000 PA and would be open to considering job shares, secondments and split roles. The Role To work within the Nursing team at TRFT and within community settings, delivering youth work support to children and young people who have asthma, epilepsy or diabetes. You will be responsible for improving the patient experience within the hospital, advocating and enabling effective transitions. You will support young people to access support within their community, working alongside youth focussed Voluntary and Community Sector organisations. You will deliver professional youth work to meet specific identified aims, outputs and outcomes. Your role will be: To develop the resilience and emotional well being of children and young people (aged 11:25).
