Certification Details Required Certifications: BLS, ACLS, TNCC, NIHSS, Trinity 9 Panel Drug Screen, Trinity Code of Conduct Form, Trinity I9, Trinity Conf. of Patient & Pty. Info. & System Security Agmt., Trinity Integrity and Audit Services Form, Trinity Specialty Specific Competency Testing, Trinity Color Vision, 2 Step PPD OR IGRA (Quantiferon), COVID-19 Vaccine, MMR, Hepatitis B, Influenza Vaccine, Varicella, Tdap, Physical/Health Statement. Additional requirements for certifications may include Trinity Profile, Trinity Conflict Of Interest Disclosure Form, Trinity Criminal Background Check, Trinity Education Verification, Trinity EPLS, Trinity GSA/OIG, Trinity NSO, Trinity System Access Form, Trinity Child and Adult Dependent Abuse Training, Trinity IA Sing Report, Trinity OFAC, and Trinity Skills Checklist based on clinical or non-clinical roles. Job Requirements American Heart Association certification is required for all direct patient care personnel. Certification to include an image of the front and back of the card. American Heart Association certification is required for RNs in CCU, ED, and PACU, MICU, SICU. Attach scanned copy of certification with expiration date to candidate's record. License/Certificate Verification must be dated within 30 days of start. State verification with NURSYS for RNs and LPNs required. Attach scanned online verification page with a machine-generated date indication. For California, must also provide a picture of HARD COPY license for RN. Additional Details 9 Panel Drug Screen must be completed within 30 days of start date, preferred within 14 days of start. Screen to include: Cocaine, Amphetamines, Opiates, 6-Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, Methadone. (NO THC). Trinity Code of Conduct Form and Trinity Health Confidentiality Agreement required. Trinity Integrity and Audit Services Form required. Trinity Specific Covid Questionnaire completion required.
