**Job Title:** Registered Nurse (RN) **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Days, 8-hour shifts, Monday-Friday (option for Tuesday-Saturday) **Guaranteed Hours:** 40 hours per week **Experience:** Minimum of 2 years **License:** Registered Nurse License **Certifications:** Basic Life Support (BLS), valid driver's license, and insurance **Must-Have:** Experience with Home Care Home Base (preferred) **Job Description:** - Provide skilled nursing care to patients in a home health setting. - Manage a caseload of approximately 5 patient visits per day, adjusted for start of care. - Perform start of care assessments, recertifications, and resumptions of care. - Educate patients and caregivers on management of conditions such as CHF, COPD, and wound care. - Manage and monitor IV-PICC lines and utilize RT equipment when necessary, including vents, CPAP, and Bi PAP. - Participate in utilization reviews and collaborate with interdisciplinary teams. - Work with a home health aide as available in the department. - Adhere to flexible scheduling with potential weekend shifts, preferably Tuesday-Saturday. - Provide coverage during holidays, with a focus on Thanksgiving and/or Christmas. - Utilize Cerner charting system for electronic documentation. - Dress code allows for any appropriate work scrubs. *Holly Haynes, the manager, is available during working hours for routine questions and after hours for urgent needs.
