Certification Details American Heart Association certification required License/Certificate Verification with a machine-generated date indication needed Fit Test, Drug Screen, Trinity Code of Conduct, Confidentiality Agreement, Integrity and Audit Services Form required Specialty specific competency testing, Color Vision test, COVID-19 Vaccine, MMR, Hepatitis B, Varicella, Tdap, Physical/Health Statement Influenza Vaccine, 2 Step PPD OR IGRA (Quantiferon/Tspot), Trinity Specific Covid Questionnaire, Trinity Skills Checklist for clinical contractors Job Details Specialty: CT Technologist Employment type: LTA (Long-Term Assignment) Contract length: 13 weeks Job Requirements American Heart Association certification is required for all direct patient care personnel Proof of vaccine, declination or exemption(with agency letterhead) is required for COVID-19 and Influenza (declination not accepted for CA or IL) Must be dated within 30 days of start for any position requiring a license or certification Attached scanned proof of vaccinations (MMR, Hepatitis B, Varicella, Tdap) with dates or primary source IgG titer with lab print-out All engaged contractors must have page 27 signed Trinity Code of Conduct Form on file Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement must be signed Trinity I9 and Conflict Of Interest Disclosure Form must be on file Attach copy of specialty exam (Trinity Specialty Specific Competency Testing) within a year Resume, Skills Checklist within a year, and 2 Professional References needed for submission Trinity Criminal Background Check, Education Verification, EPLS, GSA/OIG, NSO, and System Access Form must be completed and uploaded Additional Details Trinity Profile documentation must include Resume, Skills Checklist within a year, and 2 Professional References Professional and Personal Health History credentials must be on file for onboarding
