A company is looking for a Social Worker to assess and support the psychosocial needs of patients and families in a healthcare setting. Key Responsibilities: Assess the psychosocial status of patients and families, providing counseling and intervention as needed Maintain clinical records and collaborate with interdisciplinary teams to develop individualized care plans Manage community resources and provide information and referral services to patients and families Required Qualifications: Master's degree in Social Work 1+ year of managed care experience Clinical experience relevant to the psychosocial needs of patients and families Ability to supervise and direct professional personnel Knowledge of governmental regulations and Joint Commission standards
