Travel Nurse Source is working with Magnet Medical to find a qualified Home Health RN in Newport, Vermont, 05855 Pay Information $2,266 per week About The Position As a Home Health RN, you will be responsible for providing direct patient care in a home setting. Your primary duties include assessing patients' health status, developing and implementing nursing care plans, and collaborating with physicians and other healthcare professionals to deliver comprehensive care. You will also educate patients and their families on disease prevention, treatment options, and self-care techniques. Responsibilities: Patient Assessment: Conduct thorough initial and ongoing assessments of patients' health status, including physical, emotional, and social factors. Care Planning: Develop individualized care plans in collaboration with the patient's physician, outlining nursing interventions, goals, and expected outcomes. Clinical Procedures: Perform various clinical procedures, including wound care, medication administration, intravenous therapy, and patient education.
