A company is looking for a RN Case Manager in Rhode Island. Key Responsibilities Coordinate all aspects of patient care, including medical, behavioral, functional, and social needs Conduct needs assessments and develop care plans in collaboration with the care team Support patients during care transitions and ensure continuity of care post-hospitalization Required Qualifications, Training, and Education Current RN License in good standing in the state of Rhode Island Minimum associate degree in nursing required; bachelor's degree preferred At least 3 years of clinical experience, with a minimum of 1 year in case management Certified Case Manager (CCM) certification or commitment to complete when eligible Geriatric care experience highly desired
