A company is looking for a Registered Nurse Case Manager for a remote contract position. Key Responsibilities Lead the coordination of a multidisciplinary team to provide holistic care to members Assess, develop, implement, monitor, and evaluate individualized care plans for members Advocate for members and arrange necessary resources to meet identified needs Required Qualifications, Training, and Education Nursing Diploma or Associate's degree in nursing required; Bachelor's degree preferred 3 years of clinical nursing experience in various care settings required 1 year of case management experience in a managed care setting preferred Current, active, and unrestricted Multistate Registered Nurse license required Certification in Case Management (CCM) required within 18 months of hire
