The Mary Campbell Center is first, last, and always, home to 68 residents. The QIDP Manager is responsible for coordinating active treatment and related services for residents and ensuring they are delivered with a high degree of quality in a homelike environment where MCC residents live with respect, personal dignity, and independence to the fullness of their abilities. The QIDP Manager acts as the resident's primary advocate in coordinating services as the Qualified Intellectual Disabilities Professional (QIDP), ensuring an individualized and comprehensive active treatment program appropriate to each resident in an assigned caseload. The QIDP Manager is charged with championing the Guiding Principles and Mission of The Center in a manner that ensures compliance with State and Federal regulations. The QIDP Manager must be a positive role model who reliably works effectively, efficiently, and with respect for residents, staff, families, volunteers, professional consultants and the physical environment of The Center. MINIMUM REQUIREMENTS: The QIDP Manager at the Mary Campbell Center must 1) be a good person of high character who demonstrates compassion and understanding, consistently works with a positive attitude and advocates effectively on behalf of residents who have varying degrees of disabilities; 2) Master's Degree in Social Work, Human Services, Special Education or related field from an accredited college or university; 3) possess 3 to 5 years' experience in a social services setting working with people with disabilities (preferred); 4) exhibit strong interpersonal skills, and effective written and verbal communication skills; 5) exhibit proficiency in utilizing technology tools to manage program reporting and achieve individual, department and mission goals, and 6) be at least 21 years of age with a valid driver's license and clean driving record. ESSENTIAL FUNCTIONS: Works with the consulting psychologist, psychiatrist, outside counseling or mental health agencies, and coordinates treatment plan development and/or social support plan implementation. Provides oversight, direction and support to assigned departments. Completes annual performance appraisals for direct reports. Chairs the Mental Health Team. Coordinates the development of the social support plan, including staff consults with Case Manager/QIDP on a regular basis. Attends the Human Rights Committee and provides necessary data, maintaining and distributing minutes, coordinating meetings, etc. Follows established policy regarding informed consents of all psychiatric medications. Attends and actively participates in the following committees: Mental Health Committee, Interdisciplinary Team, HIPAA and other appropriate committees as assigned, participates in environmental rounds. Will be the HIPPA Privacy Officer. Responsible for pharmacy recs for Antipsychotics monthly. Responsible for grievance reporting. Serves as a facilitator and resource for staff to address all residents' active treatment May perform a mediation role in the resident complaint resolution process. Assists with the reporting and investigation of cases of alleged abuse, neglect and mistreatment, as assigned. Meets with residents to assess the need for follow-up treatment. Facilitates appropriate counseling as needed and provides documentation for critical incident file. Coordinates and facilitates Individual Program Plan annual review meetings according to established policy and procedures. Monitors progress of Individual Program Plan and intervenes in cases where progress is not evident, essential programs are lacking or quality is not consistent with IPP goals, including but not limited to monthly documentation reviews and program observations. Completes all documentation in a timely fashion according to established policy and procedures including progress notes as needed on all assigned residents. Effectively communicates critical information about residents' status. Maintains open communication with families, providing support and encouragement toward ongoing interaction with resident. Participates in resident admission and discharge procedures, including development of discharge summaries and post-discharge follow-up. Provides oversight to case management services for community respite participants as assigned. Monitors satisfaction of overall experience. Systematically reviews and assists residents when necessary, with personal funds, bills, spending, and escrow account statements, etc. Provides opportunity to review advanced health care directives, legal and/or financial matters. Attends continuing education experiences as approved by Facilitates and/or participates in small groups as needed to meet resident presenting needs. May be required to transport residents in the community utilizing Center vehicles. Conducts quarterly site visits for residents working or volunteering in community programs. Ensure that adequate environmental supports and assistive devices are present to promote resident independence. Supervises the Education department. Oversees the Respite Program; oversees documentation of respite programs. Acts as the liaison between Education and Case Management. Chairs the Interdisciplinary Team. Performs other duties as assigned. #admin
