Charles River Community Health - Full Time 4 day work week; 32 hours clinical 8 hours administrative Employed Average Patients seen: Carry a patient panel of a minimum of 1500 patients. Responsibilities: - Provides direct care and education to Health Center patients and ensures continuity and quality of care. - The provider will have a solid understanding of patient needs and will work collaboratively with a multidisciplinary team to co-manage primary care patients.
