Blue Sky Therapy is looking for a passionate PT to join our team! Blue Sky Therapy is looking for a Physical Therapist who is passionate about their profession, eager to work and is a team member! The PT is responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve mobility, relieve pain, increase strength, and decrease or prevent deformity of patients suffering from disease or injury. The position will entail evaluations, treatments, and discharges for a skilled nursing facility. Must be a licensed Physical Therapist! Essential Job Functions: Responsible for review of patient's charts to ensure that all patients who could benefit from intervention are screened and identified. Responsible for initial evaluations, progress notes, discharge summaries, and plan of care, when indicated. Responsible for observing and enforcing safety during all phases of patient care. Responsible for communicating, recording, and reporting to the Physician when max physical therapy benefit has been achieved. Responsible for working cooperatively with other health-care workers to provide the most efficient, economical, and comprehensive care for the patients. Responsible for utilizing time, talents and other departmental resources available to meet the objectives of the service. Responsible for delegating and instructing all or part of the patient treatment to the Physical Therapy Assistant, with supervision as needed. Responsible for interpretation of the physician referral and initial evaluation. Responsible for development of the treatment plan and program for both long and short-term goals. Responsible for selection of the appropriate portions of the program to be delegated. Responsible for reviewing all reports from the PT Assistant and signing off before being sent to the physician or filed in the patient's chart. Responsible for maintaining a high-level of knowledge in current Physical Therapy techniques by continual participation in learning background and complies with legal qualifications for continuing-ed and state licensure. Responsible for in-servicing the PT Assistant on the correct use and maintenance of equipment. Responsible for assisting the Rehab manager with administrative functions as specified by the department director, at the assigned site. Responsible for attending patient-care meetings as required by regulations of assigned agency(s). Responsible for assisting with quality assessment studies as compiled by the Rehab manager; then participates in ongoing studies as directed. Responsible for providing in-service training when asked by the Rehab manager. These in-services may be directed to other rehabilitation staff, nursing staff and students; in accordance with his/her own area of ability. Responsible for staying current on trends and developments in Physical Therapy practices, procedures, and techniques; make recommendations for improved practices and procedures. Responsible for making sure care being provided meets all Federal and local guidelines for providing skilled care. Responsible for meeting productivity goal set by Blue Sky Therapy. Responsible for timely and accurate documentation of services rendered according to billing protocol (Rehab Optima Software). Responsible for giving lectures and serving as a consultant to medical and paramedical staff and students. Responsible for ensuring patient rights to fair and equitable treatment, self determination, individuality, privacy, property and civil rights, including their rights to wage a complaint are well established and maintained at all times. Responsible for review of complaints and grievances and making oral/written reports to the Rehab manager indicating what action(s) were taken to resolve the complaint(s) or grievance(s). Responsible for abiding by the patient's decision whether or not to participate in treatment. Responsible for informing the patient of consequences of not participating in prescribed therapy and documenting such actions in the patients' record. Responsible for allowing the patient to participate in the planning and scheduling of his/her treatment. Responsible for making recommendations to the Nurse and/or attending Physician(s) for appropriate referrals to medical and paramedical personnel within the facility and community. Responsible for interacting appropriately with individuals of all ages, genders, races, socioeconomic, religious and cultural backgrounds. Responsible for completion of company's mandatory in-services (Relias). Other Skills/Abilities: Knowledge of the details and techniques needed to diagnose and treat human injuries, diseases, and deformities. Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance. Knowledge in appropriate procedures when using Physical Therapy equipment. Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. The ability to listen to and understand info and ideas presented through spoken words and sentences. The ability to maintain confidentiality. Ability to teach others how to do something and convey info effectively. Knowledge of the basic understandings of the organizational structure of the institution, basic fiscal policies including fee structures and operating costs, principals of supervision, communication, and effective interpersonal human relationships. Present a positive and professional attitude. Ability to enter and maintain accurate data in various company software programs. Knowledge of company's policies and procedures. Knowledge of basic technical skills. Physical demands: Sits, stands, bends, lifts, and moves intermittently during working hours for extended periods of time. Must be able to lift, push, pull, and move a min of 50 lbs, carry 25 lbs. Subject to hostile and emotionally upset patients, family members, personnel, and visitors. Subject to exercising patients; walking with patients. Must be able to assist in the evacuation of patients. Subject to falls, bumps from equipment, odors, assaults from hostile patients, etc., throughout the workday. Must be able to read, write, speak, and hear the English language. Subject to be involved with patients, personnel, visitors, etc., under all conditions and circumstances. Must possess the ability to deal tactfully with personnel, patients, family members, visitors, government agencies/personnel, and the general public.
