Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.Qualifications:Baccalaureate or master's program in physical therapy approved by an accredited organizationCurrent Illinois license as a Physical TherapistCurrent CPR cardValid IL driver's license and auto insurance policyWellSky (Kinnser) experience a plusIf you enjoy being part of a dynamic team using your skills to have a positive impact on the lives of others, we would very much like to hear from you!Family offers a generous compensation package with benefits to our full-time employees and is committed to attracting and retaining the best talent as an equal opportunity employer.
