Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.GREAT FLEXIBILITY. SET YOUR OWN SCHEDULE. NO WEEKENDS NECESSARY!The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait.
