Job Summary We are seeking a dedicated Per Diem Physical Therapist to provide comprehensive physical therapy services to our patients. The ideal candidate will excel in examination, evaluation, diagnosis, prognosis, and intervention, while collaborating with interdisciplinary professionals to promote prevention and wellness. This rewarding position involves coordinating patient care, conducting reexaminations, and maintaining accurate documentation to ensure optimal patient outcomes. Core Job Responsibilities Conduct thorough patient evaluations and assessments Create customized plans of care and interventions based on examination results Coordinate patient care and ensure adherence to hospital policies Engage in patient and family education to facilitate timely discharge Participate in outcomes data collection and analysis to evaluate intervention effectiveness Maintain current patient schedule and collaborate with other departments as needed Education/Experience Requirements Required: Graduate of a Bachelor of Science program in Physical Therapy New York State Physical Therapist licensure and registration Minimum one year of experience as a staff physical therapist Preferred: Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy Licensure/Certification Requirements New York State Physical Therapist licensure and registration Disclaimer We are an equal opportunity employer, and all qualified applicants will receive consideration for employment without regard to race, religion, age, gender, sexual orientation, gender identity, or disability.
