Our team of healthcare professionals positively impacts the health, well-being and quality of life for those we serve providing comprehensive, integrated, affordable healthcare and education to patients. We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers. The Patient Navigator Representative will be working to provide outreach opportunities for patients and community residents, who lack access to healthcare and health insurance and provide enrollment assistance through Marketplace for health insurance including Medicaid, Childrens Health Insurance Program (CHIP), NHHPP/PAP, and Medical discount card renewals.Responsible to keep accurate track of individuals served and provide weekly, monthly, quarterly and yearly reports as needed.
