General Job Statement The Outreach Programs Supervisor is responsible for the supervision and management of MNsure, SNAP and Outreach staff, events, and program outcomes. This position will promote and ensure staff are utilizing the whole-person integrated care model to meet the needs of guests who have insurance needs. Ensure that internal and external outreach activities and events are ongoing according to contractual agreements. This position will provide communication, support, and recommendations to the Manager to implement, improve, and evaluate all processes and services to ensure compliance with all program requirements. Minimum Qualifications: Bachelor's degree in Public Health, Human Services, or a related field and two ( 2 ) years of related experience working in a community health and human services environment. Associates degree in Public Health, Human Services, or a related field and four ( 4 ) years of related experience working in a community health and human services environment. High School Diploma or GED with six ( 6 ) years of related experience working in a community health and human services environment. Three (3) years of supervisory or program management experience overseeing grants, projects and/or services related to public health and/or social determinants of health initiatives. Minimum of three (3) years of program management experience with demonstrated skill in leadership, planning and outcome reporting, required. MNsure certified, preferred. Must obtain MNsure certification within six months of hire. Proven track record of designing and executing successful program design, implementation and meeting outcome metrics. Exceptional verbal, written, analytical, relationship building and communication skills working within a multi-cultural environment and with multiple stakeholder group. Ability to form and maintain internal and external partnerships and networks that support the mission, vision, values and goals of the organization. Knowledge of current social determinants of health and disparities, including health disparities and historical trauma and the impact to communities of color. Proficient in Microsoft Word, Excel, internet and Outlook, and client tracking database. Must have a valid Minnesota driver's license, current vehicle insurance, and ability to meet NorthPoint's driving standards. Ability to work occasional evening and weekend hours.
