A company is looking for an RN I (Oncology Care Manager) - Telephonic. Key Responsibilities: Assess patients' clinical needs and ensure appropriate levels of care Coordinate and implement care plans, monitoring services and outcomes Advocate for members and facilitate communication among healthcare teams Required Qualifications, Training, and Education: Associate's or Bachelor's degree in nursing or a health-related field, or accredited diploma nursing school Minimum of two years of clinical experience Active unrestricted NJ RN license required Proficiency in personal computers and MS Office products preferred Knowledge of case/care management principles preferred
