Occupational Therapist (OT) Case Manager

We are seeking a compassionate and dedicated Occupational Therapist Case Manager to join our team in our residential assisted living homes located throughout the southern metro area. As an OT Case Manager, you will play a vital role in improving the lives of our residents by providing personalized therapy services to enhance living skills, mobility, and promote overall well-being. Occupational Therapist Case Manager Responsibilities: • Assess residents' physical capabilities and develop individualized treatment plans. Ensuring these plans meet requirements of insurance/assistance programs (CADI, Elderly Waiver, etc.). • Implement therapeutic exercises and activities to improve functional skills, balance, and coordination. • Provide hands-on therapy and utilize modalities as needed to address specific needs and goals. • Monitor residents' progress and adjust treatment plans accordingly and ensure all documentation is complete and compliant. • Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive care. • Educate residents, their families, and our Care Partners on exercises/activities and techniques for continued progress and independence. • Participate in the development of the organizational goals, objectives and overall implementation of the program strategic plan. Physical Therapist Case Manager Qualifications: • Degree Occupational Therapy from an accredited program. • Valid state licensure as a Occupational Therapist. • CPR certification. • Prior experience working with seniors preferred. • Strong interpersonal and communication skills (English). • Compassionate and patient-centered approach to care. • Ability to work effectively both independently and as part of a multidisciplinary team. • Reliable transportation and insurance as daily travel between homes is required.
