Beverly Blvd., Montebello, CA And 10000 Lakewood Blvd., Downey CA *Will see patients at the two listed Carelon Health Medical Office locations and other offices as needed.Work Shift: 5 Days a Week, Monday - Friday: 8:00 am - 5:00 pmThe Nurse Practitioner Specialty, Nephrology is responsible for providing for the health care needs of patients in collaboration with a physician, through advanced assessment and identification of health states including abnormal conditions, diagnosing, treatment therapies and/or evaluation. Performs care in the specialty area of nephrology and patients on dialysis. How will you make an impact: * Utilizes evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines and physician instructions. * Acts independently to perform physical assessments, diagnostic functions, specialized risk assessments, and make critical decisions regarding a wide variety of health situations, including high risk and crisis situations. * Acquires patient health histories and evaluates patient, performing physical examinations. * Orders, interprets and evaluates diagnostic tests to identify and assess patients clinical problems and health care needs. * Discusses case with physician. * Formulates and documents care plan. * Prescribes medication, therapies and/or other forms of treatment. * Evaluates the patient's response to therapy. Minimum Requirements: * Requires an MS in Nursing and minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background. * Current, unrestricted RN license and NP license in the state of California is required. * Certification in a specialty or minimum of 1 year experience in the specialty area of Nephrology. * For Paragon and Carelon Health business unit, satisfactory completion of a Tuberculosis test is a requirement for this position and Hepatitis B vaccine or signed waiver.Preferred Skills, Capabilities and Experiences: * ANCC and/or AANP Board Certifications highly preferred. * Bilingual communication skills in English and Spanish. * Experience in either Primary Care or an Internal Medicine setting with adults and geriatric patient population.For candidates working in person or remotely in the below locations, the salary* range for this specific position is $132,756 to $199,134.Location: CaliforniaIn addition to your salary, Elevance Health offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
