A company is looking for a Care Coordination Nurse I - LPN. Key Responsibilities Facilitate seamless transitions for patients discharged from the Emergency Department to Home Health care Provide telephonic clinical assessments and direct patients to appropriate levels of care Work collaboratively with team members to ensure effective care coordination and follow-up Required Qualifications Active LPN license Minimum of one year experience in office procedures Minimum of one year experience with various software systems Home Health, Med Surg, or Emergency Department experience preferred Experience verifying insurance coverage and eligibility preferred
