Sunbelt Staffing, in collaboration with a prominent Nationwide Home Health Agency, is actively seeking a committed Licensed Practical Nurse (LPN) to join our team on a contractual basis. Job Details: Location: Masontown, PA (Covering Fayette & Green County, PA) Company: Sunbelt Staffing, on behalf of a Nationwide Home Health AgencyPosition Type: Contract, 13-week term with possibility of extension upon successful completionSchedule: Monday-Friday, 8:00 AM - 5:00 PM with occasional weekends and on-callCompensation: Competitive salary,.48c visit to visit mileage reimbursement, medical, dental, vision, and 401k benefits after 90 daysProductivity: 25-32 per week Travel: 100-150 miles per week Job Responsibilities: Conduct home health visits to patients homes within the territory and surrounding area. Perform comprehensive assessments of patients' health status, including physical, emotional, and environmental factors influencing their well-being.Execute personalized care plans tailored to meet the unique needs and goals of each patient, in collaboration with the healthcare team and under physician guidance.Administer skilled nursing care, encompassing wound management, medication administration, IV therapy, and patient education on disease management and preventative care measures.Maintain accurate and timely documentation utilizing Homecare Homebase software platforms.Foster interdisciplinary collaboration by actively participating in team meetings, sharing insights, and coordinating care transitions to ensure continuity and effectiveness of care delivery.Establish and maintain open lines of communication with patients, families, and caregivers, providing support, education, and advocacy to enhance patient understanding and engagement in their healthcare journey. Job Requirements: Active Pennsylvania Licensed Practical Nurse (LPN) License.Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.Minimum of 1 year of experience in home health nursing.Minimum of 1 year of experience Homecare Homebase software.Excellent clinical assessment and critical thinking skills.Strong communication and interpersonal abilities.Reliable transportation and willingness to travel within assigned territory.
