SOCIAL WORKER – CASE MANAGER SITE OF SERVICE: 633 Medical Group, Langley AFB, VA POSITION QUALIFICATION/REQUIREMENTS: Degree : Master's degree in Social Work (MSW). Education : Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE). Experience : As required to meet clinical competency requirements as specified in the service-specific credentialing instruction. Licensure: Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker. U.S. Citizenship : HCWs performing under this contract shall be U.S. citizens. English Language Requirement : The Contractor shall ensure that all HCWs providing services under this contract are able to read, write, and speak English well enough to effectively communicate. HCW Physical Capability : HCWs shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required under the contract and TO. DUTIES : Participate in Utilization Management/Case Management meetings to review and identify opportunities to improve treatment provided and recommend corrective action when problems exist. Participate in the provision of monthly in-service training for non-healthcare-practitioner members of the clinical and administrative staff on subjects germane to social work and social work CM services. Provide patient referrals. Interface with the MTF RN Case Manager(s) in the development and implementation of the Case Management Program (CMP). Develop local strategies that use the strength and disciplines of both nursing and social work to accomplish case management. Provide assistance in the development and implementation of educational programs. Additionally, the social work case manager may be required to provide those services identified under Licensed Clinical Social Worker. ADDITIONAL DUTIES : Knowledge and ability to effectively apply the following core case management Functions: a) Assessment: Identification of patients for case management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care; b) Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care; c) Facilitation: Care coordination and communication among all involved parties; and d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received. Mastery of theories, principles, and methodologies underlying psychosocial practice. Knowledge and understanding of developmental growth; dynamics of human behavior, family, and other social systems; and the impact of illness and disability on social functioning. Demonstrated abilities in program planning, implementation, and evaluation, and in conducting individual, family, group, and community assessments. Strong capability to work with service members and/or veterans and families who are experiencing a wide range of complicated mental, emotional, behavioral, physical, psychosocial, and environmental problems. Working knowledge of medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA]), and accreditation standards of Accreditation Association for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC). Working knowledge of computer applications/software to include Microsoft Office programs, MS Outlook (e-mail), and internet familiarity is required. The ability to input, extract and format data from established databases is desired. Must demonstrate ability to communicate effectively both orally and in writing. Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous. Must demonstrate ability to apply critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal and financial patient situations. Must possess organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way. Familiarity with Department of Defense (DoD), Federal, State, non-profit Healthcare systems and organizations, to include DoD Tri-Service Military Healthcare Systems, TRICARE purchased care system, Medicare, Medicaid and the Veterans Administration (VA) is desired. Experience with McKesson (InterQual) and/or Milliman Care Guidelines desired. HOURS: Duty hours will be Monday – Friday. Eight (8) hours of duty from 0730 to 1630. SPECIAL REQUIREMENTS/SKILLS Must be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Team work skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
