Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care. Take patient vitals, do assessments, document progress and report changes to RN supervisor. Assist RN in carrying out patients plan of care and teaching patient self-care techniques. Educate family on the patients plan of care, medication and home safety.
