Job details Position Objective: Efficiently and effectively triage patients in office and/or via phone. To provide a variety of nursing treatment functions to include but not limited to essment of symptoms. Performs treatment within scope of practice, prioritizes and optimizes provider schedule based off of patient need and clinical coverage, facilitates smooth patient flow, provides istance to both physician and/or clinical support staff as needed. As part of the multi-disciplinary role the Health Resource Nurse partners with the patient, patient family, internal AAHS services and/or external community resources to insure the patient is referred to the appropriate level of care. To educate the patient and/or family on the importance of treatment and/or prevention of medical conditions. To recommend treatment options within scope of practice and provides follow-up with patients and/or family if applicable Essential Position duties: Reasonable accommodations may be made to enable individuals with disabilities to perform the key functions. Provides appropriate, accurate and system-based details regarding clinical calls and inquiries. Determines over the telephone, caller's issues and needs; effectively analyzes their implications and takes appropriate and timely action to develop a recommended "Plan of Care" that produces a positive outcome for the caller. Returns all call backs in a timely manner and logs all appropriate data into triage system to ensure quality reporting. Answers phones promptly and uses knowledge to encourage residents to use appropriate AAMC/AAHS services. Provides accurate physician referrals and connections to centers of excellence. Supports health system marketing and wellness initiatives. Acts as liaison for physician to physician contact between AAMC and other health facilities; facilitate the transfer of patients to AAMC. Provides outbound call services for specific hospital initiatives to improve health outcomes of discharged patients. Attends and actively participates in community based health programs to promote Ask AAMC services and overall wellness focus. Provides connection between AAMC employees and AAMC's wellness initiatives; involved in health coaching programs. Participates in goal setting and special projects to achieve annual goals. Performs call reviews with clinical and non-clinical peers. Responds to on-line health inquiries. Educational/Background Qualifications: RN degree (BSN, ociate or Diploma) received from an licensed institution of nursing approved by the Maryland Board of Nursing. A min of 5 current years of background in med/surgery, emergency room, critical care and pediatric, home health, primary care or community health nursing. Flexibility to cover Ask AAMC's staffing qualifications. Expertise to process a high volume of calls and personal interactions in fast-paced environment. Meets performance metrics established for talk, time, average speed, etc. Skills in key wording (min of 35-40 wpm). Possess intellectual curiosity, issue solving-abilities, and the expertise to handle multiple projects with changing priorities. Expertise to stay update and competent about AAMC services and clinical changes. Required Licensure/Certification : Valid and current nursing license issued by the Maryland Board of Nursing.
